Volunteer Release and Waiver
Required for every trip participant
Every volunteer must sign this waiver before joining a mission trip. It covers travel and health risks, volunteer status, insurance responsibilities and consent to emergency treatment.

Photo and Media Consent
English / Chichewa
A bilingual form that lets patients give or decline permission for their photos or video to be used by the Foundation. It includes a witness section for verbal consent.
Team members who take photos during a trip should bring printed copies and have each form completed before any image is shared.
Volunteer Expense Report for Taxes
Volunteers often pay for their own airfare, lodging, meals and supplies. This form helps you record those unreimbursed expenses for your personal tax records. It lists the Foundation’s EIN (84-2408090) and mailing address.
